112 Medicare Part B drug codes topped $100 million in 2024. October's international pricing rule expects to reach four manufacturers

CMS published the Global Benchmark for Efficient Drug Pricing (GLOBE) Model final rule in the Federal Register on 2 October 2026. It applies an international price benchmark to Medicare Part B drugs with more than $100 million in Part B spending over 12 months, in seven drug classes, with a long list of exclusions and a waiver for manufacturers in the voluntary GENEROUS Model. CMS's Part B spending-by-drug data, as held in Svatantra.ai's signal database, shows 112 billing codes above $100 million in 2024, carrying $52.7 billion of $60.8 billion. CMS's own estimate in the final rule has four manufacturers left in the model. This note sets out the counts, the code families that are excluded by design, the five codes already headed for negotiated prices, the rule's own figures, and what the data does not show.

  3. Sidley Austin, GLOBE MFN Drug Pricing for Medicare Part B Significantly Narrowed in Final Rule (5 October 2026)
CMS expects "the number of remaining manufacturers of GLOBE Model drugs would be four," with an "expected consolidation of two" of them. Annualized savings estimate fell from approximately $2.3 billion (proposed) to approximately $80 million (final). Lookback: "the consecutive 12-month period ending six months prior to the start of the applicable calendar quarter," with quarterly inflation adjustment. CMS intends to waive mandatory participation for any quarter in which a manufacturer participates in GENEROUS; "manufacturers that have signed GENEROUS participation agreements as of August 17, 2026 will receive the waiver." Sidley attributes the four-manufacturer figure to the final rule's changes as a whole and notes CMS does not name the four.
